What Happened?

On or about January 7, 2025, US Claims became aware of suspicious activity within its network environment. Upon discovery, US Claims promptly launched an investigation and engaged legal counsel and third-party cybersecurity experts to determine the nature and scope of the incident. US Claims conducted a comprehensive review of the impacted data to determine what sensitive and/or personal information was affected and to whom it related. The investigation concluded on March 25, 2025, and revealed that certain information may have been acquired by an unauthorized individual. On April 11, 2025, US Claims filed a notice with the Maine Attorney General's office and started sending out notice letters to affected individuals.

Upon information and belief, the following types of sensitive personal and protected health information may have been compromised:
