On August 29, 2025, UI Community HomeCare filed a notice on its website and began mailing data breach notification letters to impacted individuals.

What Information Was Impacted?

Upon information and belief, the following types of sensitive personal and protected health information may have been compromised:
  • Name;
  • Social Security number;
  • Address;
  • Date of birth;
  • Phone number;
  • Medical information (medical record number, provider, type of visit, dates of service); and
  • Health insurance information.
