Upon information and belief, the following types of sensitive personal information may have been compromised:
  • First and last name in combination with at least one of;
  • Social Security number;
  • Date of birth;
  • Driver’s license number;
  • Financial account information;
  • Payment card information;
  • Geolocation data;
  • Health insurance information;
  • Medical information; and
  • Email account login information.
