What Happened?

On September 17, 2024, Physicians’ Primary Care of Southwest Florida became aware of a security incident on its IT network. Upon detection, PPC launched an investigation with third-party cybersecurity experts. The investigation revealed that an unauthorized third party gained access to PPC’s network on September 15, 2024, through a ransomware attack by the BianLian group. PPC conducted a review to determine what information was compromised and identified affected individuals on November 14, 2024. On November 14, 2024, Physicians’ Primary Care of Southwest Florida filed a notice with the U.S. Department of Health and Human Services and began sending out notice letters to the impacted individuals. The notice was later updated on July 18, 2025.

What Information Was Impacted?

The following information may have been compromised:
  • Names;
  • Social Security Numbers;
  • Driver’s license number; and
  • Protected health information.
