The investigation has determined that there was unauthorized access to PHLY’s systems. As of July 10, 2025, PHLY’s investigation is ongoing, and its systems are still being restored. PHLY has not yet confirmed the extent of the unauthorized access or the data involved.

Upon information and belief, the following types of sensitive personal and protected health information may have been compromised:

  • Other sensitive personal and protected health information.
