What Happened?

On July 21, 2025, Modernizing Medicine, Inc. became aware of potential unauthorized activity in its computer servers. Upon discovery, ModMed launched an internal investigation and engaged third-party cybersecurity experts to assess the nature and scope of the breach. The investigation revealed that between July 9 and July 10, 2025, an unauthorized actor accessed and may have acquired sensitive personal information or protected health information belonging to patients of ModMed’s podiatry clients. ModMed initiated a review of the impacted data to determine the specific types of information compromised and to identify affected individuals. Affected healthcare providers were notified on September 19, 2025. On October 17, 2025, ModMed began mailing notification letters to impacted individuals offered complimentary credit monitoring services.

What Information Was Impacted?

Upon information and belief, the following types of sensitive personal information and protected health information were compromised:
  • Name;
  • Address;
  • Date of birth;
  • Phone number;
  • Email address;
  • Bank or financial account information;
  • Health insurance information; and
  • Medical information (including medical record number, patient account number, dates of service, provider and practice name, billing and diagnostic codes, prescription/medication information, and/or diagnosis and treatment information).
