What Happened?

On January 20, 2025, Millennium Home Health Care became aware of a security incident on its computer servers. Upon detection, MHHC launched an investigation with the assistance of third-party cybersecurity experts to determine the nature and scope of the incident. The investigation determined that an unauthorized third party gained access to two of MHHC 's computer servers on January 16, 2025, and may have viewed or obtained sensitive personal and protected health information belonging to patients. MHHC conducted a comprehensive review of the impacted data to determine what information was compromised and identified affected individuals. On March 19, 2025, MHHC filed a notice with the U.S. Department of Health and Human Services Office for Civil Rights (OCR) and started sending out notice letters to the impacted individuals.

What Information Was Impacted?

Upon information and belief, the following types of sensitive personal and protected health information may have been compromised:

  • Social Security numbers,
  • Addresses,
  • Dates of birth,
  • Phone numbers, and
  • Medical conditions and care received
