What Happened?

On June 1, 2024, Maryhaven discovered unauthorized activity within its computer network. Upon discovery, Maryhaven immediately secured its systems and engaged third party cybersecurity experts to determine the nature and scope of the incident. The investigation determined that an unauthorized actor accessed Maryhaven's systems on May 30, 2024. Following a comprehensive review, Maryhaven confirmed on February 12, 2025, that sensitive personal and health information belonging to individuals was acquired during the breach. On April 9, 2025, Maryhaven filed a notice with the Department of Health and Human Services Office of Civil Rights and began notifying individuals whose information was involved.

What Information Was Impacted?

Upon information and belief, the following types of sensitive personal and protected health information may have been compromised:
  • Name,
  • Social Security number,
  • Date of birth,
  • Address,
  • Government identification numbers,
  • Medical diagnosis or condition, and
  • Health insurance information
