What Happened?

On February 6, 2025, Episource, LLC became aware of a security incident on its computer systems. Upon detection, Episource launched an investigation with the assistance of third-party cybersecurity experts to determine the nature and scope of the incident. The investigation determined that an unauthorized third party gained access to certain information within its systems between January 27, 2025, and February 6, 2025. On April 23, 2025, Episource posted a Notice of Data Breach on a dedicated response website and began sending out notice letters to the impacted individuals. The breach was subsequently disclosed to the Texas Attorney General's office on June 10, 2025.

Upon information and belief, the following types of sensitive personal and protected health information may have been compromised:

    • Addresses;
    • Phone numbers;
    • Email addresses;
    • Dates of birth;
    • Social Security numbers;
    • Health insurance information (health plans, policies, insurance companies,member/group ID numbers, Medicaid-Medicare-government payor ID numbers);
    • Medical record numbers;
    • Diagnoses;
    • Names of doctors;
    • Prescribed medications;
    • Test results;
    • Images; and
    • Care and treatment details.
