By February 26, 2026, CCHC had confirmed the scope of the incident and obtained sufficient information to proceed with notifications. hereafter, on March 24, 2026, CCHC submitted a breach notification to the New Hampshire Attorney General and began mailing notification letters to impacted individuals.

What Information Was Impacted?

Upon information and belief, the following categories of sensitive personal and protected health information may have been involved:
    • Names;
    • Social Security numbers;
    • Other sensitive personal information; and
    • Protected health information such as medical or health insurance information.
