The City conducted a comprehensive review of the impacted data to determine what information was compromised and identified affected individuals. On March 18, 2025, the City identified persons whose sensitive data was included in the impacted data. On April 14, 2025, the City of Long Beach filed a notice with the U.S. Department of Health and Human Services and started sending out notice letters to the impacted individuals.

What Information Was Impacted?

Upon information and belief, the following types of sensitive personal and protected health information may have been compromised:
  • Name;
  • Date of birth;
  • Social Security number;
  • Medical diagnosis and/or treatment information;
  • Medical provider information;
  • Medical record number;
  • Health insurance information;
  • Driver’s license or passport number;
  • Taxpayer identification number;
  • Patient account information;
  • Financial account, credit and/or debit card information; and
  • Biometric and patient account data.
