On March 14, 2025, Chord Specialty Dental Partners became aware of a suspicious activity related to an employee’s email account. Upon discovery, Chord immediately secured the account and engaged third-party cybersecurity specialists to investigate the nature and scope of the incident. The investigation revealed that unauthorized access occurred between August 19, 2024, and September 25, 2024, affecting several accounts. A comprehensive review of the impacted data was conducted, and on March 14, 2025, Chord identified individuals whose sensitive data was included in the compromised information. On the same day, Chord made a public disclosure of the incident and began notifying impacted individuals.

Upon information and belief, the following types of sensitive personal and protected health information may have been compromised:
