What Happened?

Recently, IPPC identified suspicious activity within its network environment and promptly implemented containment measures, including taking certain systems offline. With the assistance of independent cybersecurity experts, an investigation determined that an unauthorized actor gained access to IPPC’s systems between September 18, 2025, and September 19, 2025. During this timeframe, files stored on the network were accessed and copied, and may have been viewed.

Following this discovery, IPPC conducted a comprehensive review of the affected data to determine the scope of the incident and identify impacted individuals. This review concluded on or around February 9, 2026. Subsequently, IPPC publicly disclosed the incident on February 27, 2026, notified appropriate regulatory authorities and law enforcement, and began issuing notification letters to affected individuals.

What Information Was Impacted?

According to IPPC, the following categories of sensitive personal and protected health information may have been compromised:
      • Full Name;
      • Date of birth;
      • Driver’s license or other government-issued identification number(including passport);
      • Social Security number;
      • Medicare or Medicaid identification number;
      • Medical record and patient account numbers;
      • Diagnosis and treatment information;
      • Procedure and prescription information;
      • Admission and discharge dates;  
      • Health insurance information;
      • Billing and claims information;
      • Payment card and financial account information;
      • Individual taxpayer identification number;
      • Passport number; and
      • Treating or referring provider name.
